How to Enroll Your Child

Kindergarten/Transitional Kindergarten

Born on or before October 1, 2008 – Kindergarten

Born on or between October 2 to December 2, 2008 – Transitional only

Born on or between September 2 to October 1, 2008 – Option of either

Sibling Enrollment - Monday, February 4 to Thursday, February 28

Non-Sibling Enrollment - Wed., February 13 to Thursday, February 28

Step 1

Complete enrollment forms online and print out – see instructions below.

Step 2

Submit your proofs of residency and forms to Montevideo School Office along with birth certificate and immunization information as follows:

• Go to district website – www.srvusd.net
• Click tab at left “Enroll your child”
• Look at right for “ Enrollment Forms”
• Read information provided on web page. 
• Click link to “Enrollment Forms”
• This link contains all forms you will complete online. You will type information and print out the forms.

Also, plan to bring the following proofs of residence and other papers when returning enrollment forms:

• Valid Picture ID
• If Owner– Current mortgage statement, property tax bill, escrow papers or grant deed
• If Renter, rental agreement signed by both parties.
• (Also present two consecutive rental payment receipts or cancelled checks.)
• PG&E or water bill
• Additional Documentation Requested
• Birth certificate or Passport
• Immunization Record
